Rates
$160 for initial assessment, $140 for subsequent 55-minute sessions.
Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
I accept self pay, VA Community Care, Centerstone Military Services, Tricare/TricareForLife, United Health Care/Optum, PeachState/Ambetter (GA), Amerigroup, Medicare, and BlueCross BlueShield/Anthem BlueCross. Please note, I am a Tricare certified provider but have recently left the in-network contract. Please contact your insurance carrier for further information regarding coverage and benefits.
Reduced Fee
Reduced fee services are available on a limited basis.
Payment
Cash, check and all major credit cards accepted for payment. Payment is due at the time of services.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay a $40 fee prior to scheduling your next appointment.
